Family Home Visiting

Wright County Public Health works to promote the health and well-being of pregnant and parenting families. Through our family home visiting services, skilled staff members help families raise healthy children by visiting you during pregnancy or after baby is born.

What are the benefits of family home visiting?

A new baby can bring a lot of questions—we are here to provide you support as a parent. Our visits focus on supporting women to have healthy pregnancies and positive birth experiences. After baby is born, our visits help to promote your child’s health and well-being, including parenting and safety tips, and education on how your child is growing, learning, and developing. These services are free of charge and visits are flexible in how often we meet and where we meet.

Interested in getting started?
If you are interested in a home visit from a Public Health Nurse, there are multiple ways to get started: 
  • Call us at (763) 682-7468
  • Ask your doctor to let us know
  • If you go to WIC, the WIC staff can refer you

Providers and Other Referral Sources

If you are a provider or outside agency interested in making a Family Home Visiting referral on behalf of a client, please call (763) 682-7468 or fax/e-mail this Referral Form. Please notify the client of the referral prior to contacting us. Wright County has multiple home visiting programs, and we are happy to answer questions or help determine the best program for the client and their family.
