Programs Offered

Family Home Visiting - Skilled staff members are available to help families raise healthy children by visiting you throughout pregnancy and the early years of parenting.

Family Planning - The goals of this program are to encourage pregnancy planning, promote regular check-ups and testing, find and treat health problems early, and promote healthy pregnancies.

Child & Teen Checkups - Periodic examinations or screenings for individuals age birth through 20, assuring that health problems are diagnosed and treated early, before they become complex and treatment becomes more costly.

Follow Along Program - Babies and young children grow and learn at their own rate. However, some children need special help to grow up healthy and learn skills such as sitting, walking, or talking. Follow Along will help you know if your child is playing, talking, growing, moving, and acting like other children the same age. Enroll here or call/text 763-335-0280.

Help Me Grow - This program provides resources addressing developmental milestones, helps families learn if there are concerns regarding their child's growth and development and assists them in finding support.
